Add option for decision not to resuscitate/DOA

It would be good to add a few more elements around resuscitation (under skills tracker):
- At the moment you can flag interventions like ALS as 'successful' and they might have been well delivered but not achieved a sucessful outcome - in which case successful doesn't feel like the right terminology!
- can we have cases where we have sought or provided senior resuscitation advice
- can we add cases where we have made decisions to stop resuscitation either in line with JRCALC or outside the JRCALC criteria (e.g. futility, MCA 'best interests' etc)
